TMJ Pain? Try Rocabado’s 6×6 Exercise Routine

Hi, I’m Kat, a physiotherapist and co-founder of Lake Therapy. If you’ve ever experienced jaw pain, popping, or clicking when eating or talking — you’ll know how frustrating and uncomfortable it can be. This pain often stems from your temporomandibular joint (TMJ), the small but powerful hinge that connects your jaw to your skull. When it’s not functioning properly, everyday actions like chewing, yawning, or even smiling can become painful.

TMJ dysfunction can also create tension in your neck, shoulders, and posture muscles — adding to that tight, achy feeling. The good news is that relief is possible. One of the most effective, research-backed ways to ease this discomfort is through Rocabado’s 6×6 exercises for TMJ pain — a simple yet powerful series of movements designed to restore balance, control, and comfort to your jaw.

Understanding TMJ Dysfunction

Your TMJ is one of the most frequently used joints in your body. It moves every time you eat, talk, or swallow — and over time, stress, clenching, poor posture, or even injury can strain the joint and surrounding muscles.

TMJ dysfunction (sometimes called TMD) often presents as:

  • Clicking, popping, or grinding when you open your mouth

  • Jaw locking or difficulty opening wide

  • Pain in the jaw, temples, or around the ears

  • Headaches or facial tension

  • Neck and shoulder stiffness

Because the TMJ works closely with your neck and posture muscles, treating it requires a whole-body approach — and that’s exactly where Rocabado’s 6×6 exercises for TMJ pain come in.

What Are Rocabado’s 6×6 Exercises?

Developed by physiotherapist Dr Mariano Rocabado, this exercise routine focuses on realigning the jaw, strengthening supportive muscles, and improving posture. The idea is simple — six exercises, performed six times a day, with six repetitions each.

Here’s a breakdown of the Rocabado’s 6×6 exercises for TMJ pain:

  1. Tongue rest position – Gently place the tip of your tongue on the roof of your mouth, just behind your front teeth, and maintain light pressure.

  2. TMJ control during opening – Keep your tongue in that same position while you slowly open and close your mouth.

  3. Rhythmic stabilisation – With two fingers on your chin, open and close your jaw gently while maintaining tongue pressure on the palate.

  4. Neck extension – Perform small “yes” nods by lowering and lifting your chin.

  5. Shoulder posture (scapular set) – Draw your shoulder blades back and down while opening your chest.

  6. Chin tucks – Bring your chin straight back (as though making a double chin) and then relax forward again.

Each exercise supports your jaw alignment and posture, helping to ease pressure on the TMJ.

Why These Exercises Work

The beauty of Rocabado’s 6×6 exercises for TMJ pain lies in their simplicity. These movements retrain how your jaw, neck, and shoulders work together — addressing the muscle imbalances and poor habits that often cause discomfort in the first place.

Regularly practising these exercises can:

  • Reduce jaw pain and stiffness

  • Improve mouth opening and movement control

  • Decrease clicking and popping sounds

  • Ease related neck and shoulder tension

  • Support better posture and breathing mechanics

Consistency is key — small, frequent sessions throughout the day are more effective than doing them all at once.

When to Seek Professional Help
